Become a Mentor

Mentoring is one of the most meaningful ways you can volunteer because you can significantly impact the life of a young person. No special skills are needed, only the willingness to care and spend time with someone who needs you. Mentors are everyday heroes, like you and me, who transform a life or a community through their commitment to make a difference.

 

CES Mentoring Eligibility Requirements:

  • Must be 21 years old and have completed high school or the equivalent
  • Employed, retired or engaging in academic studies at an accredited institution
  • Submit an application and two letters of reference
  • Participate in a short interview with program director
  • Pass a criminal background check
  • Participate in a series of mentor training workshops
  • Make a one-year commitment to mentoring a student

You must have Acrobat 5 or later in order to complete this form on your computer. If you do not, please download the latest Adobe Reader to view this form.

  1. This form contains interactive fields that can be completed online or saved to your computer to be filled out when you open the file. Simply click on a field (i.e. - First name) and begin typing your information.
  2. Once the form is completed, save it to your desktop or at a location on your computer where you can find it. Rename the PDF file with your first and last name, i.e., John_Smith.pdf.
  3. Open a new browser window, log in to your web-based email service, i.e., hotmail, yahoo, etc. Use your email service to create a new message.
  4. Attach your PDF form and email it to mstewart@capedsupport.org.
